How to Read an Herbal Study

Most people who cite "a study showed X" read the headline, not the study. The headline was written by someone who read a press release. The press release was written by someone who may have skimmed the abstract. By the time a claim reaches you, it's gone through three or four rounds of simplification — each one a chance to lose nuance or gain exaggeration.

A few sections of a study matter more than the rest: Methods (exactly what was done — is it even the right study type for the claim?), Sample size (12 people vs. 1,200 is a very different claim), Control group (was there a comparison arm, or not — without one you can't separate the herb's effect from everything else), Outcome measures (subjective symptom score, or objective lab value?), and the Conclusion (does it actually match the data, or reach further than the results support?).
Not all studies carry equal weight, and it's not arbitrary snobbery — the research hierarchy exists because each rung controls for a specific weakness in the one below it. In vitro and animal studies tell you a compound can do something in a controlled non-human system — good for hypotheses, not for confirming human effects or dosing. Case reports describe one person — informative, sample size of one. Observational studies track real people but can't fully rule out confounding variables. Randomized controlled trials assign people randomly to treatment or control, which lets you attribute a difference to the treatment with much more confidence.
One underappreciated skill: reading an abstract's conclusion sentence critically. Researchers sometimes describe a small or marginal effect using confident, promising-sounding language — check the actual results, not just the summary sentence. And by the time a study becomes a headline, hedged language like "may be associated with" routinely becomes "proven to." Not always malicious — often just what happens when something complex gets compressed. Knowing the pattern exists is most of the defense against it.
